U.S. Passport

The Town of Mead provides Passport Acceptance Agency Service's Tuesday and Wednesday from 9:00 a.m. to 11:00 a.m. by appointment only. To book your appointment, click on the link found below. To find other facilities to process your application please visit: www.travel.state.gov

Please have forms filled out prior to coming in, as each person will only be given a 30 minute time slot. If you are missing the required forms and supporting documentation, you will have to make another appointment. 

Check or money order is required for each application.

For minors under the age of 16: both parents and the minor(s) must be present for the application process. If only one parent is available, please complete the DS-3053 form.  

For applicants ages 16-17, one parent must be present.

You must apply in person If:

  • You are applying for your first U.S. passport; or
  • You are under age 16; or
  • Your previous U.S. passport was issued when you were under age 16; or
  • Your previous U.S. passport was lost, stolen, or damaged; or
  • Your previous U.S. passport was issued more than 15 years ago; or
  • Your name has changed since your U.S. passport was issued and you are unable to legally document your name change.

Application requirements:

  1. Fill Out Form DS-11: Application For A U.S. Passport
  2. Submit Completed Form DS-11 In Person
  3. Submit Evidence of U.S. Citizenship
  4. Present Identification
  5. Submit a Photocopy of the Identification Document(s) Presented
  6. Pay the Applicable Fee
  7. Provide One Passport Photo or Photos can be taken in office for additional $15 per picture

Renewals:

Renew by mail if All of the following are true.

Your most recent U.S. Passport:

  • Is undamaged and can be submitted with your application;
  • Was issued when you were age 16 or older;
  • Was issued within the last 15 years; and
  • Was issued in your current name or you can legally document your name change with original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change.

If any of the above statements do not apply to you, you must Apply in Person using form DS-11.
