Effective December 1, 2025, the Town of Mead will be accepting appointments for in-person passport services. Appointments are accepted at the booking link below on Tuesday and Wednesday between 9:00 a.m. and 11:00 a.m. To find other facilities to process your application or with different available times, please visit: www.travel.state.gov.
Please read all of the information on this page before scheduling your appointment.
In-Person (by appointment only)
You must apply in person if:
- You are applying for your first U.S. passport, or
- You are under age 16; or
- Your previous U.S. passport was issued when you were under age 16; or
- Your previous U.S. passport was lost, stolen, or damaged; or
- Your previous U.S. passport was issued more than 15 years ago; or
- Your name has changed since your U.S. passport was issued, and you are unable to provide legal documentation of your name change.
For minors under the age of 16, both parents and the minor(s) must be present for the application process. If only one parent is available, please complete the DS-3053 form.
For applicants ages 16-17, one parent must be present.

Application requirements:
- Fill Out Form DS-11: Application For A U.S. Passport
- Submit Completed Form DS-11 In Person
- Submit Evidence of U.S. Citizenship
- Present Identification
- Submit a Photocopy of the Identification Document(s) Presented
- Pay the Applicable Fee
- Provide One Passport Photo or Photos can be taken in the office for an additional $15 per picture
Please have the forms filled out before arrival, as each person will only be given a 30-minute time slot. If you are missing the required forms and supporting documentation, you will have to make another appointment.
A check or money order is required for each application.
Please make one appointment for each family member to allow enough processing time.

Renewals:
Renew by mail if all of the following are true.
Your most recent U.S. Passport:
- Is undamaged and can be submitted with your application.
- Was issued when you were age 16 or older;
- Was issued within the last 15 years; and
- Was issued in your current name, or you can legally document your name change with an original or certified copy of your marriage certificate or a government-issued document evidencing your legal name change.
If any of the above statements do not apply to you, you must Apply in Person using form DS-11.
If you qualify for a Renewal, complete Form DS-82: Renewal Application for a U.S. Passport. You may also be eligible to renew online at www.travel.state.gov.
